Missing file on Q2 2016 upgrade

We recently upgraded our production environment 3000.1.1-rel.20+6fbf7f6.  All the building blocks work fine except for one. 

When I go into System Admin -> Building Blocks -> Installed Tools -> Questionmark Perception -> Settings:  It throws an error message "The specified resource was not found, or you do not have permission to access it."  The file it's referring to is the config.jsp file.  This happens on both our test and production servers.  Managed hosting said the file doesn't exist.  No part of this particular building block works.

 

Did anything change with the manifest requirements?  I checked through the list of API changes, etc. but I didn't see much of anything related to locations/permissions.
