Multiple spans appearing in HTML

When I look at the HTML for much of the content that I'm creating in the text editor, I've noticed that the system is inserting a series of <span></span>'s in different places where I've clicked. Today when creating a quiz question, it even placed each word and and some spaces into individual spans, <span>so</span><span> </span><span>it</span><span> </span><span>appeared</span><span> </span><span>like</span><span> </span><span>this</span><span> (without the bold). The error occurs in pretty much any type of item that uses the text editor, however I can't seem to pinpoint the root of the problem.

 

I know the error occurs (inconsistently) when:

  • Highlighting half a word
  • Clicking in the middle of a word, typing, then deleting what was typed
  • Applying <strong> or <em> to part of a word, then adding it to the other half to correct what was missed
  • Sometimes, even when just typing a regular sentence

 

I'm using:

  • Learn 9.1 (Hosted)
  • OSX 10.11.3 (Mac)
  • Firefox 45.0
  • far too much time cleaning up the HTML

Please let me know if anyone knows either the cause or a solution. Also, if more information is needed, please let me know and I will provide what I can.
